4. Medical Services, Payments & Policies

Carebridge Medical provides virtual healthcare services on a self-pay basis.

Appointments & Payments
All services require full payment prior to scheduling. Payment confirms your appointment.

Cancellation & No-Show Policy
Appointments may be canceled or rescheduled with at least 24 hours’ notice.
Cancellations made less than 24 hours before the scheduled appointment, as well as missed appointments, will result in forfeiture of the full visit fee.

Late arrivals may result in a shortened visit or require rescheduling based on provider availability.

Refund Policy
Fees are non-refundable once services are rendered or in cases of late cancellation or missed appointments.

Prescription Policy
Prescriptions are issued at the sole discretion of the physician based on clinical judgment. Carebridge Medical does not guarantee that a prescription will be provided. Controlled substances are not prescribed.

Laboratory Services
Laboratory testing may be ordered when appropriate. Laboratory fees are not included unless explicitly stated and may be billed separately by third-party providers.

5. User Conduct

You agree not to:

• Use the Site for any unlawful purpose.

• Interfere with the operation of the Site.

• Upload, post, email, or otherwise transmit any content that is unlawful, harmful, threatening, abusive, defamatory, or otherwise objectionable.
